Epidemiology

State law requires that all healthcare providers report all notifiable conditions to their state or local health department. The Brownwood/Brown County Health Department requires that all conditions be reported within 2 weeks of the initial diagnosis.

Reporting Forms
In order to make the reporting process easier, please fill out the appropriate form and fax it to the Public Health Preparedness Office. If you have any questions, please contact the PHP office.
  • Confidential Disease Report (PDF) - This reporting form collects information for multiple cases on one piece of paper when that is appropriate. It is not appropriate for disease case reports from multiple reporters to be placed on the same sheet.
  • Contaminated Sharps Injury Report - The Contaminated Sharps Injury Reporting form has been revised. There are now 2 versions of the form
  • Initial Provider Disease Report (PDF) - This reporting form is to be used for most conditions as it is designed to track the collection of the information necessary to classify a case as confirmed, probable or dropped before it is entered into the notifiable conditions' database.
